In consideration of the services of House of Honey, their agents, owner,
officers, volunteers, participants, and all other persons or entities acting
in any capacity on their behalf (hereinafter collectively referred to as
House of Honey, I hereby agree to release, indemnify, and discharge House of
Honey, on behalf of myself, my spouse, my children, my parents, my heirs,
assigns, personal representative and estate as follows:

I acknowledge that my participation in circus arts training and
instruction, including aerial arts, gymnastics and other disciplines
entails known and unanticipated risks that could result in physical or
emotional injury, paralysis, death, or damage to myself, to property, or
to third parties. I understand that such risks simply cannot be eliminated
without jeopardizing the essential qualities of the activity. The risks
include, among other things: exposing its participants to the potential
for: slips and falls and falling; rope burns; pinches, scrapes, twists and
jolts that could result in scratches, bruises, strains, sprains, cuts,
lacerations, fractures, concussions, or even severe life threatening
hazards; muscle soreness and fractures; musculoskeletal injuries including
head, neck, and back; injuries to internal organs; the negligence of other
people; my own physical condition; and the risk of emotional and
psychological injuries or physical damage associated with this activity.
Traveling to and from shows, meets and exhibitions raises the possibility
of any manner of transportation accidents. In any event, if you or your
child is injured, you or your child may require medical assistance, at
your own expense. Furthermore, House of Honey representatives have
difficult jobs to perform. They seek safety, but they are not infallible.
They might be unaware of a participant’s fitness or abilities. They might
misjudge the weather or other environmental conditions. They may give
incomplete warnings or instructions, and the equipment being used might
malfunction.

I expressly agree and promise to accept and assume all of the risks
existing in this activity. My participation in this activity is purely
voluntary, and I elect to participate in spite of the risks.

I hereby voluntarily release, forever discharge, and agree to indemnify
and hold harmless House of Honey, from any and all claims, demands, or
causes of action, which are in any way connected with my participation in
this activity or my use of House of Honey’s equipment or facilities,
including any such claims which allege negligent acts or omissions of
House of Honey.

House of Honey, or anyone acting on their behalf, be required to incur
attorney’s fees and costs to enforce this agreement, I agree to indemnify
and hold them harmless for all such fees and costs.

I certify that I have adequate insurance to cover any injury or damage I
may cause or suffer while participating, or else I agree to bear the costs
of such injury or damage myself. I further certify that I am willing to
assume the risk of any medical or physical condition I may have.

I agree that if any portion of this agreement is found to be void or
unenforceable, the remaining document shall remain in full force and
effect. By signing this document, I acknowledge that if anyone is hurt or
property is damaged during my participation in this activity, I may be
found by a court of law to have waived my right to maintain a lawsuit
against House of Honey on the basis of any claim from which I have
released them herein. I have had sufficient opportunity to read this
entire document. I have read and understood it, and I agree to be bound by

Em consideração aos serviços da House of Honey, seus agentes, proprietários,
diretores, voluntários, participantes e todas as outras pessoas ou entidades
agindo em qualquer capacidade em seu nome (doravante referidos coletivamente
como House of Honey, eu concordo em liberar, indenizar e quitar a House of
Honey, em meu nome, minha esposa, meus filhos, meus pais, meus herdeiros,
cessionários, representante pessoal e patrimônio da seguinte forma:

Reconheço que minha participação no treinamento e instrução de artes
circenses, incluindo artes aéreas, ginástica e outras disciplinas, envolve
riscos conhecidos e imprevistos que podem resultar em lesões físicas ou
emocionais, paralisia, morte ou danos a mim mesmo, à propriedade ou a
terceiros. Entendo que tais riscos simplesmente não podem ser eliminados
sem comprometer as qualidades essenciais da atividade. Os riscos incluem,
entre outras coisas: expor seus participantes ao potencial de:
escorregões e quedas e quedas; queimaduras de corda; beliscões, arranhões,
torções e solavancos que podem resultar em arranhões, contusões,
distensões, entorses, cortes, lacerações, fraturas, concussões ou até
mesmo graves riscos à vida; dor muscular e fraturas; lesões
musculoesqueléticas, incluindo cabeça, pescoço e costas; lesões em órgãos
internos; a negligência de outras pessoas; minha própria condição física;
e o risco de lesões emocionais e psicológicas ou danos físicos associados
a esta atividade. Viajar de e para shows, reuniões e exposições aumenta a
possibilidade de qualquer tipo de acidente de transporte. Em qualquer
caso, se você ou seu filho se machucar, você ou seu filho podem precisar
de assistência médica, às suas próprias custas. Além disso, os
representantes da House of Honey têm trabalhos difíceis de realizar. Eles
buscam segurança, mas não são infalíveis. Eles podem não estar cientes da
condição física ou das habilidades de um participante. Eles podem julgar
mal o tempo ou outras condições ambientais. Eles podem fornecer avisos ou
instruções incompletas e o equipamento em uso pode apresentar mau
funcionamento.

Concordo expressamente e prometo aceitar e assumir todos os riscos
existentes nesta atividade. Minha participação nesta atividade é puramente
voluntária e escolho participar apesar dos riscos.

Eu voluntariamente libero, descarto para sempre e concordo em indenizar e
isentar a House of Honey, de todas e quaisquer reivindicações, demandas ou
causas de ação, que estejam de alguma forma relacionadas com minha
participação nesta atividade ou meu uso de Equipamentos ou instalações da
House of Honey, incluindo quaisquer reivindicações que aleguem atos
negligentes ou omissões da House of Honey.

A House of Honey, ou qualquer pessoa agindo em seu nome, seja obrigada a
incorrer em honorários e custos advocatícios para fazer cumprir este
contrato, eu concordo em indenizá-los e isentá-los de todas essas taxas e
custos.

Certifico que tenho um seguro adequado para cobrir qualquer lesão ou dano
que eu possa causar ou sofrer durante a participação, ou então concordo em
arcar com os custos de tal lesão ou dano eu mesmo. Certifico ainda que
estou disposto a assumir o risco de qualquer condição médica ou física que
eu possa ter.

Concordo que se qualquer parte deste contrato for considerada nula ou
inexequível, o restante do documento permanecerá em pleno vigor e efeito.
Ao assinar este documento, reconheço que, se alguém for ferido ou a
propriedade for danificada durante minha participação nesta atividade,
poderei ser considerado por um tribunal como tendo renunciado ao meu
direito de manter uma ação judicial contra a House of Honey com base em
qualquer reivindicação da qual eu os liberei aqui. Tive oportunidade
suficiente para ler todo este documento. Eu li e entendi, e concordo em
